For a general operating request, send us a letter with the amount of your request and let us know of any changes to your programming or financial situation that may have occurred since your last grant. If the request is for a previously funded program, your letter should include the amount requested and a brief update of the status of the program. If the request is for a capital campaign, please include a narrative that explains the purpose and scope of the project, the project’s planned budget, and the status of the fundraising for it.
 

Your organization’s most recent audited financial statements of revenue and expenses and balance sheet (not your full audit) and your current budget forecast.
 

A list of other foundations from whom you have requested and/or received grants in the past year including the amounts requested or received.
 

A list of your board members and key staff.
 

A copy of your annual report, if your organization produces one
